Residents with chronic medical conditions are able to remain in the community while receiving enhanced medical supports and reduce disruptive and restrictive hospital stays.  

Provides support to people who are self-directing Community Habilitation services.  Performs life skills training and provides supervision of people who live independently in the community, in certified residential settings or with their families. Provides ongoing support and assistance to the person as outlined in their Self-Direction budget and habilitation plan.  Assists them in achieving all of their personal outcomes.
