Overview

An Intermediate Care Facility (ICF) is a community based home that provides clinical and supportive care to people with ID/DD. People living in an ICF typically require long term care, and have a higher level of need related to medical or behavioral complexities. Clinical supports are provided on-site through an interdisciplinary team process. An ICF is regulated by the NYS Department of Health through an annual auditing process by the Office of People with Developmental Disabilities (OPWDD).

Responsibilities

Primary duties and responsibilities include but not limited to:

 

  • Assists resident with personal hygienic needs by washing, toileting, feeding and showering to ensure that residents maintain good personal hygiene and to enhance their independence. If applicable, transport residents to medical appointments.  Performs light housekeeping tasks to ensure facilities are clean for residents.
  • Implements resident specific programs by following the goal book outline to ensure independence and life goals for residents
  • Assists residents in adhering to prescribed diets by ensuring correct portions are dispensed to ensure maintenance of ideal weight range.
  • If qualified, (AMAP certified), administers medication and medical procedures by following prescriptions and doctors' orders to insure proper medications and treatments given to residents
  • Maintains safety of each resident by on-going supervision and by utilizing safety techniques and adaptive equipment to protect resident from injury
  • Provides ongoing supervision at all times by being in close physical proximity to residents to insure the residents' safety and well-being
  • Implements recreational activities by assisting residents in arts and crafts, playing tapes, setting out board games, playing basketball, etc. to meet residents' recreational needs
  • Serves as a member of the inter-disciplinary team by attending and participating in all meetings to discuss residents' progress and goal achievements and to offer and gather resident specific information and ideas
  • Purchases clothing, grooming, and other personal items to meet residents daily living needs and to fulfill residents' requests to ensure appropriate appearance of resident
  • If applicable, trains new employees by covering training manual and demonstrating direct care skills to insure new employee is sufficiently prepared to handle position
  • Performs additional duties as requested by supervisor

 

Qualifications

  • High School Diploma or Equivalent preferred.
  • Qualified aNYS driver’s license required and maintained throughout employment and must meet agency insurance requirements
  • Six (6) months experience working with DDI population preferred.  Ability to understand written and oral directives/information

Physical aspects may include but are not limited to lifting and transferring of an adult/child participant.
